    Step 6: Emergency Info

In case your child needs medical treatment or our day is cut short, it is extremely important that SOMEONE IS AVAILABLE VIA PHONE AT ALL TIMES every Saturday while your child is attending Bellevue Ski School!

In the event of an emergency we will call the Primary number first, if there is no answer we will call the 2 emergency contact telephone numbers listed below in the order they are listed. All numbers may be cell phone numbers. They must ALL be different numbers.

Please list phone numbers other than your Primary phone number:

    Step 6: Concussion Policy

Please review the Concussion Information Sheet.

A concussion is a brain injury and any brain injury is serious. It is caused by a bump, blow or jolt to the head, or by a blow to another part of the body with the force transmitted to the head. It can range from mild to severe and can disrupt the way the brain normally works. Even though most concussions are mild, all concussions are potentially serious and may result in complications including prolonged brain damage and death if not recognized and managed properly.

The new “Zackery Lystedt Law” in Washington State now requires the consistent and uniform implementation of well-established “return to play” concussion guidelines that have been recommended for several years:

"a youth athlete who is suspected of sustaining a concussion or head injury in a practice or game shall be removed from competition at that time"

    and

"…may not return to play until the athlete is evaluated by a licensed health care provider trained in the evaluation and management of concussion and received WRITTEN clearance to return to play from that health care provider."

    • I understand that Bellevue Ski School will remove my student from snowsports activities if he/she has or is suspected of having a concussion injury.
    • I agree to notify Bellevue Ski School if my student has or is suspected of having a concussion injury from ANY activity.
    • I understand that my student will not be allowed to participate until Bellevue Ski School receives WRITTEN medical clearance from a qualified licensed healthcare provider regarding any actual or suspected concussion injury.
    • I further agree to review the Bellevue Ski School Concussion Info Sheet to familiarize myself and my student with observable symptoms and warning signs of concussion injury which warrant seeking medical evaluation.

    Step 8: Requests & Referral

You may request that your child be placed in a class with a friend(s), as long as the students register for the:

    • same division (e.g. High School)
    • same sport (e.g. skiing)
    • same program (e.g. Recreational Skiing)
    • same proficiency (e.g. NEVER-EVER)

Please include the name(s) of the students in the box below, and we will do our best to accommodate your request.
